What does 3g net carbohydrates per bar mean when there are more carbohydrates per serving?

Low-carb products may say "sugar-free" on the packaging, but the carbohydrates listed are usually very high. What does this mean, and how much insulin do I need as a diabetic? Carbohydrates from polyols have no effect on metabolism and do not raise blood sugar levels, so they do not need to be calculated for insulin dosage.
